This form should only be completed in the following circumstances.

If you have been informed that an earlier claim you submitted has not been processed, because information we had requested was not supplied at the correct time, whereby you would like your case to be looked at again.

Please be aware that you will still have to provide the supporting information not previously provided, and that any such documents and/or photo's can be uploaded at the end of this form.
